Hands of Hope Care Australia Pty Ltd founders have a combined total of 25 years experience in the disability sector.

We are passionate about making a positive difference in the Australian community and supporting those in need.

Our Mission: to improve the quality of life of people with a disability around Australia
and support them achieve their NDIS goals

Services

Disability support

Day Programs: fun, social and helpful

Home-based care, including showering, continence support, hoisting, peg feeding, medications, case work including annual CHAPS (Comprehensive Health Assessments) advocacy and plan writing, as well as fun and caring social interaction

Social and community support

NDIS goal implementation

Liaison, appointment support & follow-up with service providers including Support Coordinators, as well as occupations therapists, behaviour specialists, psychologists, psychiatrists, continence nurses, equipment and consumable suppliers, podiatrists, GPs, OPG, The Public Trustee, and the NDIS

Positive Behaviour Support Plan implementation and data collection

Support to evidence NDIS funding needs

Needs assessment and support to meet those needs

Qualified, experienced, caring support at our day service program, home-based care, medical and other appointments

Supporting clients to maintain and develop their family and/or other social and community relationships

NDIS goal implementation
